During one of my old playthroughs last year or the year before (I believe it was version 2.1, though possibly 3.0) I joined Stannis' retinue and served as a soldier for quite some time. I rose through the ranks until I was eventually promoted to knight (and later heavy knight). My character had started out lowborn, but after being knighted I was given the option to choose a banner! I thought that was a pretty cool touch.
I can't remember if I got it immediately, or if it was when I was on leave, or after I retired, or what, but I definitely got my banner this way. Anyway, on my current character (playing version 5.0), I tried to do the same again, but with Brynden "Blackfish" Tully as my commander. I made it to heavy knight before retiring, but this time I haven't been given the option to choose a banner.
My questions are:

Anyone else ever earn a banner this way?

Anyone know if it's a feature that's no longer offered?

If I can't get it this way, anyone know if there's a way to get a banner without starting as a noble, swearing fealty, or starting a faction? I'm RPing as a hedge knight/tourney knight and I can't be tied down. I also don't want to just join a faction and immediately quit because it's a bit gamey and out of character (plus the relation loss). The debug menu doesn't seem to offer any way to choose a banner.
