"Failed Load Order Operation" Error

Achievements not working?

I've been playing for about 30 hours or so, but I have gotten no achievements, cheats are off, I'm not using any mods and I've even tried re-installing it. Also I know for a fact there is an issue with it considering I went out of my way to complete an achievement and nothing happened when I did. Anyone know how to help?
