My character is dyinggg

My current contracted an illness and he's dying. I came prepared and have a companion with high medicine skills to keep him alive... Should I just keep him alive or let the firstborn daughter take his place and join a kingdom? Cuz my clan is t3 and not part of any kingfom yet.
