What is fast mode?

[ACOK] I haven't played since 3.1, at 5.0 is it worth a replay?

I was looking at the changelogs and it's a lot of data, it's kind of hard to read through and understand if the play experience has changed much.

Is there new weapons and armors? do you find there's anything game changing that would warrant a replay in the updates?
